Description: Caregiver Connections provides family caregiver support through support groups, workshops, and other external resource information.Caregiver support groups for families and caregivers of those affected by memory loss are held on the 2nd Tuesday of each month from 4:30 pm - 6:30 pm at Eagle River Presbyterian Church in Avon. Contact Pat Nolan, 970-328-8831, pat.nolan@eaglecounty.us.Also offers caregiver educational workshops on a bi-monthly basis. Topics may include advanced care planning, tax and legal considerations, and Caring for the Caregiver program. Also provides links for other external resources available to caregivers.
